What is PACE of the Southern Piedmont?

We are here to help you continue living in the home and community you love. PACE is a Medicare not for profit, Medicaid State option program. We serve participants 55 and older, by providing preventive and long-term care services for those requiring nursing home level of care.  

Our number one goal is to empower our aging population with the choice to live safely in the home they love. The team at PACE will work closely with you to determine program eligibility and any out of pocket cost. 


Who do we serve?

At PACE of the Southern Piedmont, we serve seniors who wish to remain in their community and  have input regarding their choices in healthcare.  Many of the participants are frail and all are assessed for the ability to remain safely in their homes with assistance and support.

To enroll in the PACE of the Southern Piedmont program you must :

  • Be 55 years or older
  • Live in the PACE service area
  • Qualify for a nursing home level of care
  • Be able to live safely in the community

We develop the most effective plan of care and provide coverage for all of your health care needs. For example, if you require ongoing medical treatment and have difficulty making yourself meals, need help with bathing or dressing or are isolated from your friends during the day, you can choose to have an assessment by the PACE team. We enable our participants to remain in the comfort of their home environment and we complement that by providing a variety of services specifically designed to meet the health and social needs of that person.
